Improper engine maintenance is being blamed for an engine malfunction and subsequent crash in Ontario a year and a half ago.

 

Flight Safety Investigators have released their report on why a Snowbird Tutor Jet crashes near Thunder Bay, Ontario in August 2005.

You may remember the team was taking part in an air show in Ontario when one of the engines malfunctioned and the plane was destroyed. The pilot ejected safely and received only minor injuries.

Well the report blames one of the first stage compressor blades that became loose and was sucked through the engine. The engine had undergone maintenance within the previous year which the report claims was not done properly.

During the investigation, other problems were identified with the Life Support Equipment such as the pilot's survival kit separating from the parachute due to incorrect stitching. As well the pilot's oxygen mask was not assembled with the correct visor and the pilot's helmet had 12 layers of paint when the standard is 5.
